INCIDENT and COMPLAINTS
WSSSA – COMPLAINT & INCIDENT Reporting Form

The WSSBL Incident and Complaint  Reporting Form is for filing any Health & Safety Incidents or concerns at the venues.

The form is also for are any complaints including, On-Court Disputes and Foul play, actions which contravene Fair Play or WSSSA By-Laws.

The form should be completed immediately post-match by all parties concerned and given to the Venue Manager to file with WSSBL administrator at schoolsportwaikato@gmail.com.

WSSSA By-laws will be used for any disputes resolution as required.
